1. Why don’t I see the USMLE application in MyIntealth?
Effective January 12, 2026, all USMLE Step exam services, including exam registration, score report delivery, and USMLE customer service, are now provided to international medical students and graduates (IMGs) by the Federation of State Medical Boards (FSMB). USMLE services are no longer available in MyIntealth. For more information and to apply for USMLE, please refer to the USMLE Service Transition Information Hub on the USMLE website.
2. Is ECFMG Certification still required for IMGs now that the USMLE Step exam services are provided by FSMB?
Yes. The Accreditation Council for Graduate Medical Education (ACGME) requires IMGs who enter ACGME-accredited residency or fellowship programs to be certified by ECFMG. ECFMG Certification is also one of the eligibility requirements for IMGs to register for and take USMLE Step 3. Medical licensing authorities in the United States require that IMGs be certified by ECFMG, among other requirements, to obtain an unrestricted license to practice medicine.
3. What services does ECFMG still provide for IMGs now that USMLE services have transitioned to FSMB?
ECFMG continues to manage the certification process, and IMGs should continue to contact ECFMG with any questions related to ECFMG Certification.

There are no changes to the requirements for ECFMG Certification as a result of the USMLE Service Transition. ECFMG continues to require IMGs to pass USMLE Step 1 and Step 2 Clinical Knowledge (CK) to meet the medical science examination requirement for ECFMG Certification.
ECFMG continues to determine whether IMGs are eligible for ECFMG Certification, and eligibility for certification continues to be a core requirement for IMGs to take USMLE. This means that, as part of the USMLE registration process for each Step, FSMB will confirm with ECFMG that IMGs are eligible for ECFMG Certification. Your Application for ECFMG Certification must be accepted before you can apply for your first Step exam.

4. Will ECFMG confirm me as eligible when I apply to FSMB for USMLE?
ECFMG determines whether IMGs are eligible for ECFMG Certification, and eligibility for ECFMG Certification is a requirement for IMGs to take USMLE. This means that, as part of the USMLE registration process for each Step exam, FSMB will confirm with ECFMG that you are eligible for ECFMG Certification.
ECFMG will share information on your eligibility for ECFMG Certification directly with FSMB at the time you apply for your examination. The eligibility requirements for examination differ depending on whether you are a student or a graduate.

*If your Application for ECFMG Certification was submitted and accepted as a student, and your medical school attendance end date is in the past, to be confirmed to FSMB as eligible for ECFMG Certification, you will need to update your Application for ECFMG Certification to either correct your attendance end date if you are still enrolled or submit your medical education credentials for verification if you have graduated.

5. I am seeing a message in FSMB’s USMLE portal that there is an issue with my USMLE eligibility status. What do I do?
If there is an issue with your USMLE eligibility status that is related to your eligibility for ECFMG Certification, the message that displays in FSMB’s USMLE portal should provide additional information and next actions. 6. How can I check the status of my Application for ECFMG Certification?
To check the status of your Application for ECFMG Certification, log in to your MyIntealth account, select My Cases from the main menu, then select the Case Number for your Application for ECFMG Certification case. Your Case Status will be located under Case Information.
Your Application for ECFMG Certification must show a Case Status of “Accepted” to be eligible to apply for a USMLE Step exam. If you are a graduate, please note that primary-source verification of your medical education credentials is required for your Application for ECFMG Certification to be accepted. The status of the credential verification cases associated with your Application for ECFMG Certification will appear under Related Cases when selecting the Case Number for your Application for ECFMG Certification to view the status of your application.
7. Where in MyIntealth can I view my MyIntealth ID and my USMLE ID?
To view your MyIntealth ID and USMLE ID, log in to your MyIntealth account, select My Profile from the main menu, and then select Identity Information from the drop-down.
8. Now that USMLE services are being provided by FSMB, do I still have to read the ECFMG Information Booklet?
Yes. An updated 2026 ECFMG Information Booklet was published on the ECFMG website on January 9, 2026. All applicants pursuing ECFMG Certification must use the most current edition of the ECFMG Information Booklet available to them at the time of inquiry, and must become familiar with, and will be subject to, the policies and procedures detailed therein.
9. If I have a credit on my Intealth Account, how can I use that credit at FSMB to pay for a USMLE exam application?
You cannot use a credit in your Intealth Financial Account to make a payment for an FSMB service. If you have a credit in your Intealth Financial Account that you do not intend to use towards our services, you can request a refund of the money. You can confirm the credit in your MyIntealth account and then follow the instructions to request a refund.
10. When registering for USMLE with FSMB, does my name need to match my name of record with Intealth?
Yes. When you start the USMLE process with FSMB, FSMB requires that your name of record exactly match your name of record with Intealth. This is necessary to enable FSMB to verify your eligibility for ECFMG Certification with Intealth’s ECFMG division. If your name as it appears on the government-issued identity document you used to create an account with FSMB does not match your name of record with Intealth, you must contact FSMB at [email protected] for further instructions.
You can check your name of record with us via MyIntealth. If the name in your Intealth record does not match your name exactly as it appears on your current, unexpired passport, you must follow our established process to request a change in your name of record. Please note: You are responsible for maintaining a current name of record with us and with FSMB. Changing your name with one entity will not automatically update your name with the other entity.
